To provide for preparation, publication, distribution, and notification of federally
mandated reports.
2.0 POLICY
2.1 Policy Statement Federally mandated reports must be prepared, publicized, and distributed to all covered
individuals in a timely manner and in compliance with Federal regulations.
2.2 Federally Mandated Reports
2.2.1. ANNUAL FIRE & SAFETY REPORT (AFSR) - Mandatory annual distribution to enrolled students, employees, prospective students,
and prospective employees; report must be accessible to the public.
2.2.2. DRUG & ALCOHOL ABUSE PREVENTION PROGRAM (DAAPP) - Mandatory annual distribution to enrolled students and employees; report must be
accessible to the public.
2.2.3. BIENNIAL REVIEW (BR) - Required every two years (minimally); distribution to enrolled students and employees;
report must also be accessible to the public.
2.3 Procedure
2.3.1 ANNUAL FIRE & SAFETY REPORT (AFSR) The previous calendar year's AFSR will be distributed annually to all enrolled students
and employees by October 1st. Distribution will occur electronically as an email message
with both a link to the updated AFSR and the updated AFSR as an attachment. Students
and employees will also receive a "campus alert" notifying them of the availability
of the AFSR online and via their University email. Additionally, students will receive
a notice of the AFSR's availability online via Canvas/Banner when they register for
subsequent term classes. Prospective students and their parents/guardians will be
informed of the availability of the AFSR on the University's website via the Admissions
Office, during orientation programs, and in The FYE classes. Prospective employees
will be informed of the availability of the AFSR on the University's website by Human
Resources during new employee orientation.
2.3.2 DRUG & ALCOHOL ABUSE PREVENTION PROGRAM (DAAPP) The DAAPP review will be distributed annually to all enrolled students and employees
by September 1st. Distribution will occur electronically as an email message with
both a link to the updated DAAPP and the updated DAAPP as an attachment. Students
and employees will also receive a "campus alert" notifying them of the availability
of the DAAPP online and via their University email. Additionally, students will receive
a notice of the availability of the DAAPP via Blackboard when they register for subsequent
term classes.
2.3.3. BIENNIAL REVIEW (BR) The biennial review will be distributed annually to all enrolled students and employees
by September 1st. Distribution will occur electronically as an email message with
both a link to the updated BR and the updated BR as an attachment. Students and employees
will also receive a "campus alert" notifying them of the availability of the BR online
and via their University email. Additionally, students will receive a notice of the
availability of the BR online via Canvas/Banner each time they register for subsequent
term classes.
3.0 AUTHORIZATION
3.1 ANNUAL FIRE & SAFETY REPORT (AFSR) The President’s Designee has the authority to annually modify or approve changes to
this policy or the attachments as mandated by federal regulations with the approval
of the designated campus official, designated by the President.
3.2 DRUG & ALCOHOL ABUSE PREVENTION PROGRAM (DAAPP) & BIENNIAL REVIEW (BR) The Vice President of Student Affairs has the authority to annually modify or approve
changes to this policy or the attachments as mandated by federal regulations subsequent
to review by the University Policy Committee and with the approval of the President
or his/her designee.
2017 Equity in Athletics Disclosure Act (EADA) Review The Equity in Athletics Disclosure Act requires co-educational institutions of postsecondary
education that participate in a Title IV, federal student financial assistance program,
and have an intercollegiate athletic program, to prepare an annual report to the Department
of Education on athletic participation, staffing, and revenues and expenses, by men's
and women's teams. The Department will use this information in preparing its required
